important Information from our School Nurses

Hello! To say we have missed seeing everyone at school would be a big understatement. We both became nurses to take care of people, so it has been especially tough the past two months to not get to see the students every day. However, we have utilized this time in quarantine to catch up on some much-needed paperwork in the nurse’s office. We wanted to give some updates to finish up the end of the school year:


ATTENTION: Please read if you have a child that takes prescription medication during the school day and/or has asthma.


If your child takes prescription medication during the school day, we need to have a NEW medication form completed by your child’s physician. These forms need to be completed every single year; we cannot give any prescribed medication without a current signed medication form on file.


Also, if your child has asthma, we are required to have an Asthma Action Plan filled out and signed by your physician every single school year. Just a reminder, an inhaler is considered a prescription medicine and needs to have a doctor’s note/action plan to be carried at school. We have placed some blank prescription medication forms and Asthma Action Plans in the N5 foyer, which will be available for the remainder of the school year. Please take one to keep for your child’s next doctor’s visit so that it can be completed prior to the start of school in the fall.  




  • Vaccine Requirements:

Among many other tasks, we have looked up every single South Adams student’s vaccination records on CHIRP (Indiana database for immunizations) and cross checked any records on file at school. Out of over 1200 students, (yes, we really checked every single student!!) we found SEVERAL still missing one or more required vaccinations. We understand the COVID 19 pandemic has not made getting these vaccinations an easy process. To help alleviate unnecessary calls and/or visits to physician offices and pharmacies, we will be mailing letters to ONLY those students who still NEED required vaccinations, with the exception of current 5th and 11th graders. If you do not receive one of these letters in the next few weeks, your child should be up-to-date and current on his or her vaccinations. However, we strongly encourage you to double check your child’s records at home. If you feel there is a discrepancy, please don’t hesitate to reach out via email or phone.

*****Just a reminder, ALL students K-12th grades are now required to have 2 doses of Hepatitis A vaccine; this is a 2-dose series that needs to be administered at least 6 months apart. *****

If your child is up-to-date with his or her immunizations, there should be no need for further vaccines UNLESS they fall into one of the following categories:

Incoming KINDERGARTENERS (Medical packets will be available at round-up in July, but you may pick them up sooner in the N5 foyer until the end of the school year.)

  • DTaP (Diphtheria, Tetanus, & Pertussis)-5 doses
  • Polio- 4 doses
  • Hepatitis B-3 doses
  • Hepatitis A-2 doses
  • MMR- (Measles, Mumps, & Rubella)- 2 doses
  • Varicella (Chickenpox)-2 doses

Incoming 6TH GRADERS (ALL current 5th graders will receive a letter explaining this in detail; the MCV4 cannot be given before the age of 11.)

  • Meningococcal/MCV4 (Meningitis)
  • Tdap (Tetanus, Diptheria, and Pertussis)


Incoming 12TH GRADERS: (ALL current 11th graders will receive a letter explaining this in more detail.)

  • 2nd dose Meningococcal/MCV4 (Meningitis)


We are asking that you please provide a paper copy after your child receives the required vaccinations. Not all physician offices utilize CHIRP, and sometimes, like all technology, the CHIRP website is down so not usable during that office visit. We never want children to get duplicate vaccinations but if it wasn’t entered into CHIRP at the doctor’s office or pharmacy and a paper copy wasn’t provided to the school nurse, it appears to not be done. With over 1200 students to keep track of, this ensures a smooth process for the school nurses to make sure everyone is up-to-date.
