• ALL families who wish to be considered for breakfast/lunch, home internet/device assistance, and other important benefits should complete the free/reduced application.


    The application only needs to be completed one time per household.


    Please have the following information ready to make completion simple: All household family members names, SA students’ DOB, grade, household income information, telephone to be contacted, social security number (if available) and assistance information if applicable (Food stamps number, etc).

    To complete the application- please complete one of the following ways.

    1. Complete online at: https://www.mymealtime.com/Apps/Register.aspx
    2. Print the PDF application below and mail to 1075 Starfire Way, Berne, IN 46711 or turn into school office. 
    3. Pick up a hard copy of application from the SA school office and return upon completion or mail to 1075 Starfire Way, Berne, IN 46711.
